lexical_cast uploaded

Code, basic tests, and initial documentation for lexical_cast has been
loaded into the lexical_cast folder. This supersedes stream_cast.

It includes some of the suggestions from the list plus an open approach
intended to make headway/compromise on some other issues (eg exceptions
and precision specification).
