If Jeremy agrees, I'd be open to a patch which separated all of the
specialized adaptors from iterator_adaptor, and, at least for the time
being, provided iterator_adaptors.hpp including everything as a backwards
compatibility measure.

Obviously, the patch would need to have documentation tweaks as well.
