[Boost-docs] Generating documentation?

I have a couple of questions:

1) If I go to the directory $BOOST_ROOT/libs/foreach/doc and do "bjam 
--v2", I get HTML documentation but minus the image files (like for 
"Prev", "Up", etc.). How do I get the image files into the proper location?

2) If I go to the directory $BOOST_ROOT/libs/foreach/doc and do "bjam 
--v2", the HTML documentation appears inside the subdirectory "html". So 
I figured that if I do "bjam --v2 pdf", the PDF documentation would 
appear inside the subdirectory "pdf", but no such directory appeared. At 
first I thought no document was generated at all due to errors reported 
by FOP. But further investigation reveals that there is a PDF document, 
but it is buried deep inside the directory hierachy $BOOST_ROOT/bin.v2/.....

Was my expectation unreasonable and, if so, why?
