iterator adaptors

Status update:

I've just begun to review the iterator adaptors code that Jeremy put in the
vault in mid-June, expanding on the work that I had already done. The code
looks really good, but the documentation is particularly terriffic (great
job, Jeremy!)  I suggest you have a quick look at the docs, if you don't
know what it's about already.
