type traits docs issues

Just a few issues/inconsistencies I've noticed:

1) 'remove_cv' class template is not described in the "Transformations
Between Types" section's table, although 'remove_const' and
'remove_volitale' templates are; in fact, the only places where the template
is mentioned are the "Type Traits Headers" section's table and the
'Copy_example.cpp' source code;

2) a typo: "Note that the >>>maco<<<
BOOST_BROKEN_COMPILER_TYPE_TRAITS_SPECIALIZATION evaluates to nothing on
those compilers that do support partial specialisation."

3) there are two different spellings of the "specialization" word across the
docs - the British one and the American one, that is also used in the
standard; I have nothing against the first one, but IMO consistency here
would be nice :).
