Bind is now in CVS

I've added mem_fn.hpp, bind.hpp, and libs/bind/* to the CVS.

Notable changes:

* mem_fun renamed to mem_fn;
* improved documentation;
* nested binds are now supported;
* monolithic test (bind_test.cpp) added.

I have left the placeholders in a global unnamed namespace (at least for
now) in an attempt to gather more information about the real-world chances
of a collision.
