Iterator adaptor documentation

There is something that puzzles me almost every time I look in the iterator 
adaptor documentation. Policies have to use the undocumented non-const base() 
member of iterator_adaptor in order to do their job. As a result the reader 
(at least me) is unable to fathom how the default_policies work unless he has 
a look in the header.

I think a small comment on this would make the interaction much easier to 
understand.
