Thanks to Aleksey's tip, the last incompatibility with MSVC is fixed.

the file in the vault, tagged version 1.42, 
compiles on 
gcc-3.0
MSVC6 - sp5
Metrowerks CWP 7.2

In addition to the MSVC fix, some minor changes were applied  (comments,
small changes so as to suppress some warnings, headers renamed.)

And this version is at last suitable as a final submission.
