[boost] [shifted_ptr] Documentation roots

Greetings Boost:

I have uploaded a new version having minor fixes but I have also included 
some basic documentation describing the shifted_ptr internals.  The former 
API reference doc has moved to its given "api" subdirectory.  I apologize 
for the non-standard format of the documentation files; this is temporary.

There isn't much left to be done on the functionality side of the project 
besides adding pool allocation for internal objects.  Let's put an end to 
that application crash era:
