After more googling, I found this on the QNX web site:

__QNX__
    The target is a QNX operating system (QNX 4 or QNX Neutrino).
__QNXNTO__
    The target is the QNX Neutrino operating system.

So it looks like __QNX__ casts a wider net than __QNXNTO__
