Subject: [Boost-docs] Boostbook DTD clash

I note that using Docbook 4.5 DTD I see:

when building quickbook/boostbook docs, looks like the <code> element
defined by the Boostbook DTD was introduced in Docbook-4.3 so the two DTD's
now clash. Is there any way we can fix this? Maybe we should just require
Docbook-4.3 and later and remove the <code> definition from Boostbook?
