The attached patch allows doxygen2boostbook to
handle friend functions defined inline.  It adds a new
section called "friend functions" if there are any such
functions in the class.  Since I couldn't see a way to
distinguish friends that are actually defined inline from
other friends, they will be ignored if they don't have
a documentation comment.
