It'd probably be better to use the UTF-8 characters rather than entities.

FWIW, there already is an 'ifdef':
http://www.boost.org/doc/html/quickbook/syntax.html#quickbook.syntax.phrase.cond
It'd be possible to define a 'html' or 'boostbook' macro.
