I've got what looks like a similar problem re-building previously OK docs under Windows.

local-boost-root =  I:\boost_trunk  (recently updated)
quickbook.quickbook-to-boostbook ..\bin.v2\doc\msvc-9.0\debug\threading-multi\quick_auto_dox_index.xml

I've tried adding

using xstlproc ;

but makes no difference.

For pdf generation:
