Autoindex gets run after everything (doxygen included) is converted to 
Docbook - so it can index Doxygen content as well as the quickbook stuff - 
it also handles manually inserted <indexterm>'s so if the Doxygen stage can 
add these more precisely than autoindex can so much the better....
