One thing I spotted is that you output the index data as if they are 
doubles, but I think they are integer types.

I.e., the double in 

> std::copy(index1_data.begin(), index1_data.end(),
>    std::ostream_iterator<double>(cout, "\n"));

is definitely wrong.
