 Specify that [#named_anchors] are global in the documentation.

 so [link named_anchor some link text] works globally in the whole
 Quickbook document.

 This behaviour is OK - though it could cause some trouble - what if there
 are two anchors with the same name in different sections (fairly likely).

 So it needs a warning that this could cause trouble.

 OTOH it *is* useful to have global links without having to have a section
 just to provide a link.
