 Running the Spirit2 tests on VC9 with warning level 4 produces quite a few
 warnings, mostly 4100 (unreferenced parameter) and 4512 (assignment
 operator could not be generated).

 Attached is a patch to silence the C4100 warnings. I'll look at the others
 seperately.
