 I ran into this problem from quickbook which for the markup
 [headerref path/to/header.hpp header]
 generates

 <headername alt="path/to/header.hpp">header</headername>

 Unfortunately, boostbook ignores the alt and looks for a file
 called "header".
 This is not consistent with the behavior of classname e.g.
