 Currently templates are phrase elements (including block templates), so
 they are always generated inside a paragraph, but they sometimes generated
 content that shouldn't be in a paragraph. I plan to fix this in my spirit
 2 port of quickbook.

 This also causes problems for imported code as it's implemented using
 templates.

 This has come up previously in #1280, #1281.
