 This is the remaining issue from #2153. Boostbook has `<methodname>` and
 `<classname>` elements with different semantics to the the docbook
 elements with the same name. They're not in the dtd for this reason, and
 not documented. Should possibly rename the `<*name>` elements to something
 more sensible.
