 Before I commit this change to a release I would like to be sure that the
 current trunk changes are not covering what might actually be an error in
 VC++ 2010. It may well be that VC is incorrect in requiring that an
 iterator have output_iterator_tag as its category. The standard seems
 ambiguous about this.
