 boost/thread/pthread/mutex.hpp ignores nearly all failures from pthread_*
 functions. Functions include pthread_mutex_lock and pthread_mutex_unlock.

 From a design perspective, it might be prudent to forgo pthreads and
 simply use atomic operations when available.
