Sigh... it's probably trivial if you understand XSL well enough - both
the htmlhelp and webhelp customisation layers (in the docbook xsl
stylesheets) do exactly this as I understand it. But it would need
someone familiar enough with XSL to implement this for our XSL
customisation layer. That person is not me BTW.
