For DocBook, you can use the DocBook XSL stylesheets, available at:

        http://docbook.sourceforge.net/

For FO, you'll need some kind of FO translator. Apache FOP is one  
open-source solution, but we've typically had bad luck getting all of  
the Boost documentation to format properly with it. There are several  
commercial solutions for generating PDFs from FO files.
