You need to use an intermediate variable.
You could also do this to prevent copying:
v.resize(v.size()+1);
v[v.size()-1][0] = 4;
v[v.size()-1][1] = 9;
v[v.size()-1][2] = 11;
v[v.size()-1][3] = 19;
