Thanks for giving me a credit here, Dave, but I am the wrong person for it,
at least regarding the "classical" (no templates involved :) form of the
technique. I think D&E was the first source where the discussed way of
simulating named function parameters was described, so I would give all the
credit to Bjarne. I still remember how much I was excited when I read that
chapter of his excellent book :). Obviously, in some sense the (reinvented?)
named _template_ parameters technique are just a translation of the D&E idea
to a new programming dimension (generic/template metaprogramming). I am a
big proponent of both of them, though :)
