interlocked (atomic) stuff is non-portable ("portable" version 
with mutex has completely different semantics)
POSIX CV impl. is incorrect (see comp.programming.threads)
mutex impl. is looking quite strange (2 "real" mutexes + CV ??)
how about return code checks?
