About "dlw_util.zip" 11

This is version 11, the latest post-review release.  I removed the 
previous post-review release (10) and the release that was 
formally reviewed (8) a couple of days ago.  You can send me 
e-mail if you need one of those.

The fixed-constructor class templates were renamed to put the 
number at the end in numeral form.  This should make it easier 
to mass-produce other templates with higher constructor 
argument counts (with the preprocessor library).

A source of confusion during the formal review was that I'm 
usually verbose in my examples, so some reviewers thought all 
my qualifications on some names were mandatory.  I redid 
the test example to use minimal qualification.

The documentation was changed to fit both of the above 
changes.
