loki::typelist VC6 port

I was inspired by Andrei's recent boost submission to port
boost::loki::typelists to compilers lcaking partial specialization. In
particular, my port now appears to pass the test suite under MSVC++ 6.0sp5.
The code started out cleanly, but after staying up most of the night
battling internal compiler errors battles it is somewhat less than
beautiful.  If there is any interest I can clean things up a bit put
something in the files section.
