My only objection to the fix is that it breaks code which depended on the
bug, and it does so at a distance. In large legacy systems written for MSVC,
there will be at least some such code. If we were to start using prefix and
suffix files to bracket all our headers we could think about doing it for
boost code, but personally I'm not ready for that change.
