Boost.Format formal review

I think that Samuel Krempp's Format submission should be accepted by Boost.

The issues I spotted have already been identified by other reviewers.

I'd particularly like to restate that I think there should be only one main 
interface, and unless it can deliver exact printf behavior, it is better to 
have that interface not be an exact clone of printf's.

Enough issues have come up that it would be useful if the final version was 
posted for a mini-review before integration into the web site.
