Agreed we should deprecate BOOST_NO_STD_UNORDERED - but I would set it if 
*either* of BOOST_NO_CXX11_HDR_UNORDERED_SET or 
BOOST_NO_CXX11_HDR_UNORDERED_MAP  is set (since it implies both).
