I seemed to remember this constructor was explicit. If it isn't, it should
be. If you want convenience, I would think it sufficient for
make_chained_output_iterator to take multiple function objects. Or provide
some component that can conveniently compose multiple function objects...
(Boost.Bind and Boost.Phoenix can do this, I think, but it could probably
be specialized to reduce the syntactic weight).
