OK here's the problem, this line:

#<xsl:param>fop1.extensions=0

is commented out in Trunk, which means there is *both* XEP and FOP document 
outline info placed in the .fo file.

If you build with FOP then this doesn't cause an error - it just ignores the 
XEP extensions it doesn't understand - but if you build with XEP it 
understands both the FOP and the XEP outline info and doesn't know which to 
use - hence the error.
