OK, I just removed the support for escapes in simple markup. I never
noticed before that if you use \<, \>, \& in quickbook it doesn't xml
encode them. I guess that might be deliberate, but it doesn't feel
right to me. But that isn't something that should be changed without a
version switch (as this problem demonstrated).
