[Boost-testing] [test] [thread] including unit_test_suite_ex.hpp fails to compile

One of the Boost.Thread library test programs (test_mutex.cpp) fails
to compile due to the inclusion of the header file
<boost/test/unit_test_suite_ex.hpp>.  This file includes another file
which does not exist:

This test seems to be failing across several platforms.
