Clean-up to Do Before Release

1.  Since the "tie"-ing code has moved from the utility library, the forward
declaration of the "boost::tied" class template should be removed from
<boost/utility_fwd.hpp>.

2.  The HTML file that lists all the libraries needs some edits.

A.  Under the "miscellaneous" section, the listing of the utility library
should only mention "noncopyable," since the other items in that library are
already given in the other listings that library gets in other sections.

B.  The base-from-member idiom library needs to be listed.
