[boost] Output Formatters (was I/O Formatting Manipulators)

I have posted updates to the source to support state objects to support 
facilities such as position information, while allowing outputters to be 
constant:

I have moved the documentation over to XML, providing a HTML version of it. 
In doing this, I have made a few revisions and updates to it, including 
documentation for state objects.
